Neuropsychology Evaluations

Rates and Insurance Questions

A neuropsychological evaluation can provide valuable information about your child’s neurocognitive strengths and weaknesses and assist in the development of best practices and evidenced based instruction.   While reading is the focus at The Bend Learning Center, often there may be related issues of attention, memory and processing speed that are contributing to the learning challenges.  Dr. Sondra Marshall offers individualized evaluation packages depending upon your child’s needs, assessment questions and financial considerations.

Insurance Questions:

A neuropsychological evaluation typically requires many hours of professional time.  At BLC we do not contract with any insurance companies and therefore our assessments are considered an out of network service and reimbursed by your unique insurance company accordingly.  Please note, Dr. Sondra Marshall uses her own TAX ID so regardless of what your insurance company says, she is NOT in-network when she completes private evaluations through BLC. We provide an invoice with all necessary documentation to submit to insurance.   Should you want to look into what your company may reimburse, the CPT numbers are the following:

  • 90791
  • 96132
  • 96133
  • 96116
  • 96136
  • 96137

The diagnosis under which the codes are billed is unique to your child and will be determined by the question being asked as well as previous evaluation results.

Different Packages:

The hourly rate for neuropsychological testing services is $250.  All evaluations include time spent interviewing the individual and/or important people in his or her life, administering neuropsychological/achievement tests, analyzing/interpreting the data, preparing a detailed report, and debriefing the results.

  1. Because of the time required, the total cost for all services included in one of the comprehensive assessments is typically between $2500 and $4,000.  These are often complicated evaluations which seek to provide differential diagnosis of neurocognitive, neurodevelopmental, academic and social/emotional/behavioral issues.  These are diagnostic evaluations and inform treatment decisions.  Testing questions typically include determining if a child/youth has ADHD, Autism Spectrum Disorder, Complex Mood Disorders, as well as learning disorders.  Diagnosis can be very valuable to inform treatment pathways and provide important and necessary documentation when applying for some social service supports or access to certain interventions.

a.    These assessments include the following:

                                               i.     Intake with parents/caregivers

ii.     Review of all records available and interviews with other professionals

iii.     Behavioral measures completed by parents/teachers

iv.     Face to face testing with child/student

                                             v.     Scoring/interpretation/report writing

                                            vi.     Debrief with parent/caregiver

vii.     Review with School as requested

 2.    An evaluation to determine if a student has a learning disability such as Dyslexia typically varies between $2,000-2500 depending upon age of the child.  These evaluations focus on academic specific areas that inform about processing strengths and weaknesses in reading, writing and/or math.  It is imperative that the child’s educational team provide information about school functioning in efforts to best inform the evaluation process.

 3.    Perhaps the full Dyslexia assessment feels too overwhelming to start. I can begin with a learning disability screening which provides a 30,000 foot view of how a child is performing in reading, writing and math relative to same age peers. It is not meant to be diagnostic but rather can guide intervention planning.  This cost will vary depending upon age of child between $750-1500.

 4.    Should you be interested in an assessment to determine intellectual and/or academic functioning to assist with informed placement decision making or perhaps for the purpose of determining giftedness, these evaluations cost between $1,200 for children kindergarten-age and younger; $1,500 for children in grades 1 through 12.

 5.    I can also provide updated testing for access to accommodations for college entrance exams (SAT/ACT) or in an institute of higher learning such as Community College or University.  Typically, there has to be a history of testing and access to accommodations in elementary or high school, or a history of a diagnosis such as ADHD, Anxiety or Learning Disorder for accommodations to be provided at an institute of higher learning. These evaluations are personalized with costs between $1,500-3,500 depending upon previous evaluations, review of records and documentation required.
